Patient forms will be sent from my medical records patient portal: Elation Health Passport. (Discovery calls do not have additional forms that need to be completed). Please submit any forms before your appointment.

All telehealth visits and discovery calls are via zoom. You will receive the zoom link before your scheduled appointment time.
