* Shorter, 25 minute telehealth appointments are only available when scheduled within 6 weeks of your previous appointment; otherwise upgraded to a 50-minute visit. Additional time: $50 per 10 minutes if a visit runs long.

** Portal messaging is meant for clarification of your current plan. New concerns or symptoms require a visit for best care.

*** Beyond the listed timeline, an appointment is required prior to further messaging support.

Membership is nonrefundable, but can be cancelled for the upcoming year at any point up until the renewal date. Simply message our practice through the secure patient portal to request membership changes.
